Before you do anything else, you should run 12V directly to the fan to make sure its working properly. If the fan is good and the fuse is not ... Unplug it and check the wires for power. One should have power. If neither has power with the ignition on then check the fuses. If one does have power then jump it to the other. If the fan comes on, the temp switch is bad. Unhook the linkage; if it still pops out of gear, then it's an internal problem.
